Program overview

The program offers support and guidance to patients, while facilitating an open dialogue about options for renal replacement therapy, including transplantation and home dialysis.

About the workshop

Each workshop includes take-home materials, a video, and a one-on-one session with a clinical educator. Videos and printed materials are available in English and Spanish.

Workshop format

    Nurse provides a brief introduction on the specific topic.

    Patient views an educational video on a specific topic.

  3. 3

    Clinical educator meets with patient for 15-20 minutes to review materials, answer questions, set goals, and develop an action plan.

    Patient receives printed educational materials to take home and an action plan pertaining to the topic.
