Honed over a decade of experience, and operating at a competitive scale, our unique care model allows us to credibly take on total cost for the complex and costly kidney patient population.
We use a two-pronged approach that involves patient care and provider enablement to manage the highest-need, highest-spend patients and provide preventative interventions that effectively slow kidney disease progression. Both are supported by our connections into patients’ existing care environments through our partnerships with the largest value-based nephrology network and over 2,600 dialysis clinics nationwide.
- Our Total Patient Care Service includes four programs: Care Management, Transition Management, Medical Care Extension, and Clinical Documentation. We work in partnership with nephrologists and PCPs to deliver the right care at the right time to the right patients.
- Our Provider Enablement Service incorporates support and tools that create alignment between providers and our care team, including interoperability with Acumen® Epic®, improved workflows, and provider incentives to further value-based care, all of which deepen our impact and promote sustainable change in the system by ultimately driving physician behavior change.
